I have used Teamspeak[1] in the past for gaming.  IT works quite well and is free, but closed source.
All you need is a mic and speakers (or a headset).  I have been with groups with 40+ people from all over N.America.  It has nice admin features similar to IRC. ie admins, limited admin, block by IP, allow by IP, "listen only" etc...
